Annual Liability Release Form
The following form is needed for children to attend any of the following activities sponsored by LWC:
- camps
- retreats
- any event involving transporation
- sleepovers
- etc.
This form is valid from September 1st, 2009 until September 30th, 2010. If you have any questions regarding this form or other concerns, please contact Pastor Jeremiah.
